Christian Health Center is nestled between historic Old Louisville and downtown and is easy to get to by interstate and bus routes. Our Transitional Rehab to Home unit provides: • Spacious private and semi-private rooms with home-like amenities • Short-term physical, occupational and speech therapy • Skilled nursing care • Patient education Our goal is to help patients recuperate, gain strength and return home as quickly as possible! But until then, our vibrant activity programs, delicious meals and full-time chaplain help make this temporary stay a great one. We accept Medicare, Medicaid and private insurance. In addition to Transitional Rehab to Home care, Christian Health Center provides specialized memory care, long-term skilled nursing care and outpatient rehab services. As part of Christian Care Communities, we also offer cozy and affordable independent and assisted living apartments at Friendship House and Chapel House.
